TREATMENT FOR DISEASES OF GASTROINTESTINAL TRACT: LIBERAL DIET FOR PEPTIC ULCER
Many physicians now prescribe a so-called liberal diet, which is essentially a normal diet, at the very beginning of therapy. However, if the ulcer is located in the esophagus or if the ulcer is bleeding a more conservative approach is recommended.
The liberal diet may follow the description of the normal diet but with emphasis on these points:
1. Based upon a diet history, individualize the diet according to the patient’s food habits and preferences. Explain fully the reasons for any modifications that should be made in this pattern.
2. Regulate energy intake according to weight status; those who have lost weight should have a daily caloric intake about 500 kcal above their normal requirement.
3. Include recommended amounts of foods from the four food groups to ensure nutritive adequacy.
4. Use moderation in food selection; avoid excessive use of any one food group; for example, milk, high-fiber foods, and so on.
5. Allow all foods which the patient tolerates, except for items listed in number 6. Some can tolerate all strongly flavored vegetables while others experience discomfort with the inclusion of one or more of this class.
6. Omit alcohol, tobacco; meat soups, meat extractives; pepper, chili powder, mustard, and nutmeg. Small amounts of coffee or tea with milk are usually permitted.
7. Eat small meals and include midmorning, mid-afternoon, and evening snacks. Small meals reduce the distention of the stomach and thus reduce acid flow. Protein in each of the six feedings temporarily neutralizes acid.
8. Rest before and after meals. Eat slowly in a pleasant environment. Avoid conversations that are emotionally upsetting.
