Capoten (Captopril)
Arterial hypertension (mono therapy and combined one), backward heart failure, cardiomyopathy, dysfunction of left ventricle while steady state for the patients after having had myocardial infarction, diabetic nephropathy against diabetes of type 1.

Action

Captopril decreases a general peripheral vascular resistance and reduces preload and afterload. The drug decreases tension in right atrium and in lesser circulation. Captopril decreases general peripheral vascular resistance.

Posology and Administration

When there is arterial hypertension it is administered 12.5-50 mg 2-3 times a day. When there is renovascular hypertension it is administered 12.5 mg 3 times/daily. When there is chronic heart failure it is administered 12.5-25 mg 3 times/daily. The maximal daily dose is 150 mg. Captopril is to be taken fasting.

Counterindications

The drug should not be taken if there is hypersensitivity to Captopril, if there is liability to edemas, if there is narrowing of renal artery (bilateral and unilateral); after kidney transplantation; if there is narrowing of an aortal valve or a mitral one; likewise in pregnancy and in the period of breastfeeding.

Should Be Taken with Precautions, if

The possibility of the use of Captopril by children is to be determined by the attending doctor. Therapy of patients of more than 65 years of age should be carried out with a special caution. Before the use of the drug it is necessary to check the kidney function. In the beginning of the therapy it is necessary to control the value of arterial tension and in patients of a risk group special laboratory parameters (compromised renal function patients and collagen disease patients).
If while Captopril therapy there emerge such symptoms as fever, increase of lymph nodes and/or angina it should be done a white corpuscles blood test.
Alcoholic beverages consumption should be avoided (alcohol performance enhancement is possible), likewise should be avoided salt in abundant (it weakens Captopril performance).

Overdosing

Symptoms: significant drop of arterial tension, stroke, myocardial infarction, labored breath and harried respiration, angioedema.

Side Effects

On the part of nervous system and sense organs: fatigability, dizziness, headache, CNS depression, drowsiness, confusion of consciousness, depression, choking sensation, oxygen starvation, convulsions, benumbed or pricky sensation in the limbs, vision and/or sense of smell disorders.
On the part of cardio-vascular system and blood: low arterial tension, stenocardia, myocardial infarction, heart rhythm disorder, acute stroke, increase in size of lymph nodes, anemia, pain in the breast, disorders of blood composition, clots in pulmonary artery and in general blood current.
On the part of respiratory system: bronchospasm, labored breath, unproductive dry cough.
On the part of intestinal tract organs: loss of appetite, disorder of taste, stomatitis, cankers of mucus coating of mouth cavity and stomach, dry mouth cavity, swelling of gums, aglutition, nausea, vomiting, digestive upset, excessive flatulence, pain in the stomach, constipation or diarrhea, pancreatitis, liver injury.
On the part of genitourinary system: compromised kidney function, quantitative reduction of urination, urinary albumin excretion, impotency.
On the part of skin inguments: blush, rash, itching.
Allergic reactions: Stevens-Johnson syndrome, hives, Quinckes edema, allergic shock etc.
Others: elevation of the bodys temperature, chilly sensation.

Pregnancy and Lactation

Before the use of Captopril women of child-bearing age should make a pregnancy test. While Captoptil therapy women should be accurate in following of contraception rules. In case if during Captopril therapy pregnancy nevertheless occurs, it is necessary, on a doctors recommendation, to substitute this drug for another one less dangerous for the baby, as Captopril therapy, especially in the last 6 months of pregnancy may be harmful for the baby.
