What about cortisone
This to some people is a dreaded word, as when the drug was first produced it was thought to be a complete cure for virtually everything and consequently was often prescribed irresponsibly for too long a period. Side-effects developed and left everyone very suspicious of it. I must stress, however, that used correctly for short, sharp courses it is a most valuable and effective drug. Its main action is to reduce the swelling of the lining of the airways, and this can be a life-saver. Furthermore if used prophylactically, via an inhaler, cortisone can prevent asthma attacks developing. The absorption into the rest of the body is negligible, so no unwanted effects can develop. In severe attacks cortisone is given either as a single injection or in tablet form for a few days. Research has shown that it takes at least five weeks of continuous treatment to produce any significant ill-effects. So please do not worry if your child is put on to treatment containing cortisone.
Are the medications addictive
They most certainly are not, and if your child seems to require more and more of the inhalers then a review of the treatment is needed. It usually means there is either poor inhaler technique or the dose needs raising as the child grows older.
Will my child become immune to the medication
There is no loss of effect with continuous usage, so again if your child seems to need larger doses then the treatment needs reviewing, especially inhaler technique.
My child always has a nasty cough during an attack. Would you recommend using a cough medicine
As the airways are very irritable it does make the child cough continuously. Coughing is a very strong reflex action whose main purpose is to protect the lungs from dust and grit. To completely dampen this reflex would leave the lungs wide open to any foreign particles which are breathed in. Unfortunately most cough medicines do not affect this reflex and therefore have little effect. The real key in controlling the cough is to settle the wheezing.
What is the maximum number of medications my child should take
A newly-registered patient of mine came for review of her treatment at our asthma clinic and I found she was on eleven different types of medicines, tablets and inhalers. In my experience it is always possible to control asthma using a bronchodilator inhaler, a cortisone inhaler, a nebulizer and a course of cortisone tablets. Therefore the maximum number of different drugs, even in a severe attack, is four and for routine use or in milder episodes should only be one or two.
Is it safe to exceed the stated dose
With drugs taken via an inhaler there is a wide safety margin so in general it is not dangerous to take extra. The dose of the inhaler is only one tenth that of a corresponding tablet and in fact is only one fiftieth of that produced via the nebulizer. It is not the worry of toxic effects in increasing the dose that is important but rather the indication that a change in treatment is needed.
How can you tell if the inhalers are nearly empty
If you are using a diskhaler or rotahaler then it is easy to see how many disks or capsules are left. The sealed pressurized inhalers are more difficult as the drug is in a vapour form. My son devised a trick to give some indication. By placing the canister of the inhaler into a bowl of water the position of it shows how much remains. If the canister sinks then it is nearly full, if it points directly down it is half full and if it floats flat then the canister is empty. Of course it is always wise to have a spare inhaler readily available in case of emergency. Not only do they become empty just at the vital moment but they also have the irritating habit of suddenly refusing to work, even if they are nearly full.
